Astúries
La Unitat d’Epidemiologia Mol·lecular de l’Institut Universitari d’Oncologia de la Universitat d’Oviedo, va iniciar a Astúries, a l’any 2004, el reclutament d’una cohort de nounats de la població del Área Sanitaria III, quan les mares estaven de 12 setmanes, per tal d’estudiar el paper de les exposicions ambientals en el desenvolupament (neuroconductual i antropomètric) i la salut (principalment respiratòria) dels infants en el marc de l’estudi INMA (INfància i Medi Ambient) que finança l’Instituto de Salud Carlos III. Es tracta d’un estudi observacional de cohorts que segueix embarassades de la població general des del primer trimestre fins al part i als nounats des del naixement fins a l’adolescència. Les dones, seleccionades durant el període 2004 – 2006, havien de ser residents de l`Area III d’Astúries, tenir 16 anys o més i complir altres criteris d’inclusió, com ara: que fos un embaràs únic, que no hi hagués cap impediment per a una bona comunicació i que no patissin cap malaltia crònica.
L’àrea sanitària III d’Astúries engloba els consells de Avilés, Corvera, Castrillón, Gozón, Cudillero, Pravia, Muros de Nalón, i Soto del Barco amb capçalera a l’Hospital San Agustín de Avilés. Aquesta àrea inclou el consell d’Avilés i es caracteritza per un grau elevat d’industrialització que ha ofert una bona oportunitat per crear una cohort de seguiment des de l’embaràs; la quantitat de població va permetre reclutar en dos anys el nombre d’embarassades requerit; la integració de les seves estructures sanitàries va facilitar que totes les embarassades anessin a un únic centre d’atenció secundària per a la primera visita i la primera ecografia i un únic hospital per al part.
La cohort reclutada presenta característiques comunes de els embarassos en el nostre entorn (edat mitjana: 31.82 anys, ocupació durant l’embaràs: 70.51%, tabaquisme en el tercer trimestre d’embaràs: 17.76%), encara que les participants tenien una certa selecció cap a una millor educació i suport social que la mitjana: educació superior a la primària: 81.85%, família biparental en néixer l’infant: 99.38%. Tot i això, la variabilitat en les exposicions, tant a la contaminació com a la dieta, assegura que es tracta d’un marc contextual de gran valor per a l’estudi proposat.
La informació recollida inclou exposició individual a contaminació atmosfèrica durant l’embaràs, creixement intrauterí i postnatal, desenvolupament mental a l’any i mig, i dieta i nutrients durant l’embaràs i a l’any i mig de edat. També es va recollir sang de la mare, de cordó i llet materna. Actualment s’està completant el seguiment als 18 mesos d’edat, amb una resposta del 92 per cent.
