En octubre del año 2003 se llevó a cabo el estudio piloto para la creación de la cohorte INMA en Valencia. En febrero de 2004 comenzó el reclutamiento de la cohorte que se prolongó hasta junio de 2005. Se reclutaron 855 mujeres pertenecientes a los departamentos de salud 6 (34 municipios) y 7 (dos barrios de Valencia) de la provincia de Valencia. Las embarazadas se seleccionaron por muestreo consecutivo de entre todas las del área de estudio que acudieron al cribado poblacional del síndrome de Down que se realiza entre la 10-13 semanas de gestación en el hospital la Fe, hospital de referencia de la zona, y que cumplían los criterios de inclusión: (1) Tener al menos 16 años cumplidos; (2) Estar en la semana 10-13 de gestación; (3) Embarazo único; (4) Intención de continuar el seguimiento y dar a luz en los centros de referencia correspondientes; (5) No tener ningún impedimento para la comunicación y (6) No padecer enfermedad crónica previa al embarazo.
Durante el embarazo se realizó el seguimiento en la semana 12 y 32, donde se obtuvieron muestras biológicas (sangre, uñas y orina) y datos relacionados con la dieta, el trabajo y la exposición a contaminantes.
Los recién nacidos se siguieron a lo largo de la infancia. Se realizó una visita al año de vida en la que se evaluaba el desarrollo neurológico, su antropometría, salud respiratoria, alimentación y exposiciones ambientales. Adicionalmente, a la mitad de los niños se les midió los niveles de contaminantes atmosféricos en su casa (dentro y fuera), instalándose dosímetros durante 15 días. A los 2 años se realizó una entrevista telefónica y a los 4 se les citó de nuevo en el Hospital la Fe para evaluar los hábitos dietéticos, crecimiento y obtención de muestras biológicas (orina, pelo y sangre) para la medición de nutrientes y exposición a contaminantes ambientales.
