La Unidad de Epidemiología Molecular del Instituto Universitario de Oncología de la Universidad de Oviedo, inicia en Asturias en el año 2004 en concreto en la semana 12 de embarazo una cohorte de recién nacidos de la población del Área Sanitaria III con el objetivo de estudiar el papel de las exposiciones ambientales en el desarrollo (neuro-conductual y antropométrico) y la salud (principalmente respiratoria) en el marco de la red INMA (Infancia y Medio Ambiente) que financia el Instituto de Salud Carlos III. Se trata de un estudio observacional de cohortes que sigue embarazadas de la población general desde el primer trimestre hasta el parto y a sus hijos desde el nacimiento hasta la adolescencia. Las mujeres debían ser residentes en el Área III de Asturias, con edad igual o superior a 16 años, reclutadas en el periodo 2004-2006. Otros criterios de inclusión fueron: embarazo único, que no hubiese impedimento en la comunicación y no padecer ninguna enfermedad crónica.
El área sanitaria III de Asturias engloba los concejos de Avilés, Corvera, Castrillón, Gozón, Cudillero, Pravia, Muros de Nalón, y Soto del Barco con cabecera en el Hospital San Agustín de Avilés. Esta área incluye el concejo de Avilés y contorno con un alto nivel de industrialización que ha ofrecido una buena oportunidad para crear una cohorte desde el embarazo: el tamaño de población permitió reclutar en 2 años el número de embarazadas requeridas; la integración de su estructura sanitaria facilitó que todas las embarazadas atendieran un único centro de atención secundaria para la primera visita y la primera ecografía y un único hospital para el parto.
La cohorte reclutada presenta características comunes de los embarazos en nuestro entorno (edad media 31.82 años, ocupación durante el embarazo 70.51%, tabaquismo en el tercer trimestre de embarazo 17.76%), si bien las participantes tenían una cierta selección hacia una mejor educación y soporte social que la media: educación superior a la primaria 81.85%, familia bi-parental al nacimiento del niño 99.38%. Sin embargo la variabilidad en las exposiciones, tanto de contaminación como de dieta, asegura que se trata de un marco contextual de gran valor para el estudio propuesto.
La información recogida incluye exposición individual a contaminación atmosférica durante el embarazo, crecimiento intra-uterino y post-natal, desarrollo mental al año y medio, y dieta y nutrientes durante el embarazo y al año y medio de edad. También se recogió sangre de la madre, de cordón y leche materna. Actualmente, se está completando el seguimiento a los 18 meses de edad, con una respuesta del 92%.
